Killeen, TX (January 10, 2021): At approximately 5:29 p.m., Killeen Police Department officers were dispatched to the 1600 block of Carrollton Avenue in reference to a 911 call in reference to a psychiatric call. Upon the officer’s arrival, he encountered an emotionally distressed man. The officer initially used his conducted energy weapon, which was ineffective, and then discharged his duty weapon during the encounter, striking the subject. The male was transported by paramedics to the Carl R. Darnall Army Medical Center and later succumbed to his injuries. He was pronounced deceased by medical personnel and Justice of the Peace Bill Cooke. Killeen, TX (January 13, 2021): The deceased male has been identified as 52-year-old Patrick Lynn Warren, Sr. of Killeen. The officer involved, Officer Reynaldo Contreras, is a 5-year veteran of the department. He remains on administrative leave pending the results of the investigation. In addition to the investigation being conducted by the Texas Rangers, the Killeen Police Department is conducting a separate Internal Affairs investigation. The Los Angeles County Sheriff's Office released body cameras (3) surveillance video, 911 call, maps and picture about a officer Involved Shooting that occurred in Lancaster, CA last February. On February 6, 2024, at about 3:45 a.m., a male who was later identified as 38-year-old Jonathan Scott Foster of Lancaster entered a convenience store in the 42700 block of 20th street west in Lancaster. Foster armed with a machete smashed the glass door of the store and attacked the clerck. Deputies responded but Foster was not located. Deputies notified nearby businesses of the incident and asked anyone who saw someone matching Foster's description to call 911. At 9:28 a.m., Lancaster Station deputies responded to a business in the 43500 block of 20th Street West, Lancaster, regarding a call of a business disturbance. The caller stated a male, armed with a machete, was at the location. Deputies located Foster standing outside the front entrance to the business with a machete in his hand and an additional machete affixed to his waistband. He was given numerous commands to drop the machete but refused to comply. After several minutes Foster walked into the open business which was occupied by numerous customers. Deputies on scene fired a taser multiple times and launched several 40MM foam batons at Foster, but they had minimal effect in part because Foster was wearing a tick jacket. After the deployment of a third less-lethal weapon attempt, Foster charged at deputies while still armed with the machete and a deputy-involved shooting occurred. 3 deputies discharged their service weapons. Foster sustained several gunshot wounds to his upper and lower torso. Deputies rendered first aid until the arrival of Los Angeles County Fire Department paramedics. Foster was transported to a local hospital where he was pronounced dead. Two machetes, approximately 24 inches in length, were recovered at the scene. There were no other people injured in the incident. Los Angeles Sheriff's Department LASD released surveillance camera videos, pictures and details of a Deputy Involved Shooting at Santa Clarita Valley Station on March 1-2020. Los Angeles County Sheriff's Homicide Bureau detectives responded to Santa Clarita Valley Sheriff’s Station, located at 23740 Magic Mountain Parkway in Valencia, to investigate the circumstances surrounding a deputy-involved shooting that occurred on Sunday, March 1, 2020 at approximately 2:29 p.m. Homicide Bureau Lieutenant Alfred addressed the media and stated that Santa Clarita Valley Sheriff’s deputies were driving out of the station’s secured parking lot when the armed suspect, a male Hispanic adult, suddenly appeared walking on foot. The deputies attempted to detain the armed suspect at gunpoint and ordered the armed suspect numerous times to drop the firearm. The suspect refused and during the attempts to get the armed suspect to comply and cooperate, a deputy-involved shooting occurred. One deputy fired two rounds, striking the suspect. The suspect was transported to a local hospital where he was pronounced dead. A firearm was recovered at the scene and it turned out to be a replica gun, Airsoft guns are replica toy weapons used in airsoft sports that shoot plastic bullets. No deputies were injured. It was imperative that PPD conduct a thorough and in-depth internal investigation into this incident. Today, PPD notified police Det. Daniel Siemen that he has been terminated for violating the department’s use of deadly force policy. The action comes after the completion of an internal investigation into the shooting death of Tymar Crawford. Det. Siemen was the only officer to discharge his weapon during the incident. “We work hard to develop and maintain trust in our officers by all members of our community, and if an officer violates that trust, we take appropriate action,” Police Chief Tommi Lyter said. “We want residents to know that this incident is not reflective of the training, policies, procedures, or culture of the Pensacola Police Department. The men and women who work in the department also live in this community and have a passion for providing professional, high-quality police service to the citizens of Pensacola.” The internal investigation followed an independent investigation by the Florida Department of Law Enforcement, the results of which are with the State Attorney’s Office.
