Carbonic anhydrase is an enzyme in the proximal convoluted tubule that helps dissociate the carbonic acid (H2CO3) into protons (H+) and bicarbonate (HCO3-). Then, the H+ will be secreted in urine while the HCO3- will be reabsorbed into the blood..

Carbonic anhydrase inhibitors (e.g. acetazolamide) will inhibit the previous step leading to loss of bicarbonate in the urine (alkalinization of urine) and reabsorption of protons into blood (acidification of blood leading to metabolic acidosis).
