Tissue plasminogen activator (tPA) converts plasminogen into plasmin. Plasmin dissolves the fibrin thrombus into FDPs and D-dimer (d-dimer is produced via dissolution of the stabilized fibrin).
There are two types of plasminogen activator; Urokinase type plasminogen activator (UPA), and tissue-type plasminogen activator (tPA).

Iron deficiency anemia (IDA) is the most common cause of anemia worldwide.
Iron deficiency anemia starts as normocytic (normal MCV), and then become microcytic (low MCV).
Iron absorption occurs mainly in the duodenum, just after passing through the stomach and its hydrochloric acid (HCl).
There are 4 main subtypes of microcytic anemia:-
1) iron deficiency anemia (IDA)
2) anemia of chronic disease (ACD)
3) sideroblastic anemia
4) thalassemia.
I've discussed all of them in my videos...Check my hematology playlist: https://goo.gl/KxAu6F

How is iron absorbed, and what's the big deal?
Iron is related to anemia (big time).
We use iron studies all the time.
This video will make you understand all of this.
It's a 2-part subject, so please watch my next video on iron.

Hepatorenal syndrome is a difficult disease to understand and the pathophysiology is still controversial.
The liver secretes Nitric oxide (NO) into the circulation, which will lead to vasodilatation of blood vessels and hypotension.
Without sufficient pressure, the kidney is severely affected.
So, a problem in the liver (hepato) leads to a problem in the kidney (renal), therefore; hepatorenal syndrome.
The main issues are; vasoconstriction (of the efferent arteriole) and vasodilation (all over the body due to the nitric oxide).
Hope it helps!
And as I said, it is not fully understood yet.

In my opinion, the fastest way to raise your score on USMLE is flashcards.
There is a program called firecracker, it’s really fun, they made flashcards for you and they repeat questions based on YOUR OWN specific knowledge of each one, they may be offering DISCOUNTS now http://mbsy.co/gcHHP

This set of flashcards covers “the most likely diagnosis”… a must have for any exam, I have gone over them more than 3 times to review and refresh, that’s how good they are. https://goo.gl/FZqTsn
If you want flashcards for pharmacology, these cards rock, trust me, I have tried them myself, https://goo.gl/Hxdr9y
As of flashcards for Diagnostic tests, these are especially helpful for USMLE step 2 and COMLEX step 2, https://goo.gl/awWm7B
Last, here is a set of flashcards about "physical findings” which is great for USMLE step 1, step 2CS, CK or step 3 https://goo.gl/pJqw5Z
