Set near Port St. Lucie, Florida in 2011, the story follows Jotaro Kujo's estranged daughter, Jolyne Cujoh, who is framed for a murder and ends up in Green Dolphin Street Jail. Jotaro, who was hunting down rogue Stand users and divorced from Jolyne's mother, comes to visit her, but is rebuffed due to his absence from Jolyne's life. Jotaro informs his daughter that a disciple of DIO's framed her so that he could kill her in prison, and urges her to escape. This plan goes awry when a Stand named Whitesnake uses its power to remove Jotaro's Stand and memories, in the form of discs. Revealing his love for his daughter, Jotaro sinks into a deathlike state, and Jolyne must find a way to recover the discs from Whitesnake's user, a Catholic priest known as Enrico Pucci, who also serves as the prison's chaplain. Along the way, she picks up allies such as Emporio Alnino, a boy born in prison; Ermes Costello, a young woman seeking revenge for her sister; Narciso Anasui, who soon falls into unrequited love for Jolyne; and Weather Report, an amnesiac whose memory Disc had been stolen by Pucci and named himself after his Stand. She also meets Foo Fighters (shortened to F.F.), a hybrid of a Stand and a plankton colony made sentient via Whitesnake and used her Stand abilities to possess the corpse of a recently-deceased female inmate.

Pucci met and idolized DIO sometime before the events of Part 3. While in DIO's company, DIO spoke of "Heaven" and how to create a "perfect world" when certain conditions were met. Although it has been 23 years since DIO's death, Pucci, keeping DIO to his word, proceeds with his plan to realize this "perfect world", which involves absorbing the remnants of DIO's bones in the form of a green homunculus called the Green Baby, and arriving at a predestined place on the night of the new moon. The process of DIO's plan was outlined in his diary, which Jotaro had previously read and burnt after killing DIO. This caused Pucci to target Jotaro to steal his memories, while possibly mobilizing DIO's ex-followers, making him responsible for Jotaro leaving his family to hunt them down.

Jolyne retrieves Jotaro's Stand Disc, but Pucci reveals that he only needs Jotaro's memory Disc to complete his plan. He awakens the Green Baby despite Jolyne and her friends' efforts, and successfully absorbs it. Foo Fighters and Anasui are mortally wounded in the failed effort to stop Pucci, but Foo Fighters uses the last of her strength to save Anasui's life and retrieve Jotaro's memory Disc. Jolyne and her allies escape from prison, and Jolyne sends both of Jotaro's Discs to the Speedwagon Foundation.
