Update: everything working perfectly! I just had to ignore the Monoprice supplied Cura links which aren't good for the Mac. Detail here: https://www.youtube.com/watch?v=mvqBNdiVYIQ
I can print the supplied cat.gcode perfectly... It looks great! But the gcode I'm producing from Cura 2.3.0-beta are no good... the bottom layer won't start properly.

Instead of calibrating each ESC individually, you can calibrate all of them at the same time. Take a breadboard, and put some straight pins such that you have a row of 5 pins in each row, for 3 contiguous rows. Attach the Rx and 4 ESCs such that all 5 positive leads are wired together, all 5 negative leads are wired together, and all 5 signal leads are wired together.
Then you can follow the calibration method for your ESCs, and all of them will be calibrated at once. For many ESCs, the procedure is: (1) remove props. (2) attach wires as per above. (3) THR stick to max. (4) attach power. hear initial tones. (4) THR stick to min. hear final tones indicating endpoints have been calibrated.
