When an office building collapses, House has to work fast to diagnose a young woman, Megan (Bevin Prince), who has managed to survive the disaster. Without a team, House talks through his ideas with a janitor at Princeton-Plainsboro. As House persists in diagnosing Megan by himself, he soon realizes the case is not what it appears to be and that solitude may not be the answer.
Tension between Israel and Qumar escalates while Bartlet is preparing for the Presidential debate. Both the questions raised in debate preparation and those present lead to flashbacks to the administration's first weeks in office. Toby tries to get his pregnant ex-wife to remarry him.
Saxon asks Vogel to rehabilitate him, as she helped Dexter. Dexter gives Batista his 2 weeks notice, and tells Debra he's moving to Argentina with Hannah and Harrison. Clayton becomes suspicious that Dexter is leaving Miami. Debra returns to Miami Metro and rekindles her relationship with Quinn, who has broken up with Jamie. Despite her desire to help Saxon, Vogel decides to betray him after watching the video of Zach's murder. After a treadmill accident, Hannah must take Harrison to the ER, but a nurse recognizes her and puts Clayton on her trail. Vogel brings Saxon to her home, but her nervousness makes him suspicious. When Dexter arrives, Saxon murders Vogel and escapes.
House and Cuddy face a widespread outbreak on their plane back from a symposium in Singapore, while Wilson and the rest of the team treat a 58-year old woman (Jenny O'Hara) with constant seizures who is hiding secrets about her lifestyle.
Dexter moves in with Debra so she can monitor and protect him. Dexter discovers Louis was the one who cancelled his credit cards and confronts him. While investigating Mike Anderson's murder, Quinn gets friendly with a Russian stripper named Nadia (Katia Winter). LaGuerta secretly investigates the blood slide, comparing it to the slides from the Bay Harbor Butcher case. Meanwhile, Ukrainian mob boss, Isaak Sirko (Ray Stevenson), arrives in Miami to look for Viktor Baskov. Also, Wayne Randall (Daniel Buran), a spree killer from 15 years ago, decides to reveal where he buried his victims.
The episode opens with the American delegation to Israel in the Gaza Strip. A British photojournalist, Colin Ayres, has paired up with Donna Moss, and takes pictures of her right before an explosion flips over the Suburban carrying her and Admiral Fitzwallace. In a series of flashbacks, Colin shows Donna the human side of the conflict, taking her to visit both Palestinians and Israelis, and Josh reads the emails Donna sends; in the present, Josh and Toby attempt to find out what happened. Two Congressmen and Admiral Fitzwallace are confirmed dead, and an injured Donna is flown to Germany. Leo lets Josh go to be by her side.
Results are coming in from around the country. Both Charlie and the First Lady are concerned for the President's health. The stunning result in the California 47th could have serious implications for Sam. Donna, while trying to "swap votes," meets a new White House military aide. The President wins re-election in a landslide.
House systematically begins eliminating his new team candidates, until he is approached by a fighter pilot named Greta (Essence Atkins), a candidate for NASA's astronaut training program. Greta suffers from a neurological disorder, where she converts sounds to visual images. Knowing NASA will reject any possibility of her becoming an astronaut if they knew of her problem, Greta begs House to treat her in secret. Meanwhile, House is ruffled when he thinks he sees Cameron, Chase and Foreman in the hospital hallways.
The hospital is hosting an oncology benefit poker tournament when a six-year-old boy (Carter Page) is brought in exhibiting symptoms identical to those of a patient House had twelve years ago. House is convinced the boy's case is identical and he can predict the course of the young patient's illness, which ended in the first patient's death.
Six months after the season 3 finale, Dexter is now living with three kids and a wife in a new home. Along with his new son Harrison, the complexities of being a suburban father of three is making it difficult for Dexter to concentrate on his work. Dexter's lack of sleep results in a murderer being released because Dexter brought the wrong case file to court, irritating Quinn. Quinn begins a relationship with reporter Christine Hill. A recently retired Agent Lundy has returned to finally capture a shadowy serial killer known as the "Trinity Killer", who has been at large for 30 years and always kills three people every year. After a call from Rita, Dexter quickly kills the released murderer but totals his car after falling asleep at the wheel.
