It is Christmas at Princeton-Plainsboro and Wilson has a present for House: he and Detective Tritter have struck a deal and House has three days to accept it. Cuddy receives a patient (Kacie Borrowman) afflicted with dwarfism, who has a variety of symptoms and is recovering from a recently collapsed lung. Cuddy is eventually forced to make a difficult and potentially life-threatening choice between her patient and House's well-being.
House and his team investigate the mysterious poisoning of high-school student Matt Davis (John Patrick Amedori), until another teen is brought in with all of the same symptoms but almost nothing else in common with Matt. Eventually it is discovered that both youths bought very cheap pairs of jeans at a car boot sale. The jeans were exposed to high doses of pesticides. Meanwhile, House has an old lady hit on him who turns out to have Neurosyphilis.
House and the team take on the case of Alice (Alyssa Shafer), a young girl with pancreatitis. Since her divorced parents cannot agree on how to proceed with her treatment and will not let House bully them into making a decision, House's only option is to take them to court and let a judge rule on the matter. Meanwhile, House's reduced access to Vicodin is beginning to take its toll and he asks Cuddy for more, but instead of writing a prescription, she strictly rations his pills.
Travis tasks Beth with unleashing Wormwood poison gas in the Miami Metro squad room, but she's stopped by Dexter, saving everyone's life. Debra has dinner with Chief Matthews where she confronts him about being implicated in the dead call girl case. When Matthews is forced to retire, she realizes that LaGuerta has been manipulating her throughout the case. Dexter stages a fake Doomsday Killer tableau in order to draw out Travis, but is captured himself and becomes a part in the final tableau, the "Lake of Fire". Debra's therapist prompts her to consider whether she may actually be in love with her adopted brother.
Speltzer is arrested by police and admits to the murders, but his case is dismissed on a technicality and he is set free. As Dexter pursues Speltzer, he is captured and trapped in Speltzer's labyrinth, but manages to escape. Meanwhile, Isaak convinces the club's bartender to be a scapegoat for Anderson's murder to get the police off them. Also, Hannah McKay visits Miami Metro to help with the investigation of Wayne Randall's murders. Dexter finally catches and kills Speltzer.
The religious murderers stage a spectacular crime scene recalling the Book of Revelation, using pieces of the runner’s corpse sewn to pieces from mannequins to form the Four Horsemen of the Apocalypse. The police department officially starts to chase the "Doomsday Killer". Meanwhile, Harrison needs emergency surgery, which forces Dexter to reconsider his views about faith with the help of Brother Sam.
Following a harrowing chapter in the nation's history, the White House celebrates the Fourth of July, but most of the staff is not in a cheerful mood. C.J. is moved by the resignation letter of a senior State Department diplomat who could not stomach the Shareef assassination. Bartlet endures the painful process of nominating a candidate for Vice President. His first choice, Secretary of State Lewis Berryhill, can't be approved and he and his staff are unimpressed with the "compromise candidate" they end up with, reflecting on how recent events have emboldened the Republicans. Meanwhile, while the reclusive First Lady tends to personal matters and expresses anger towards her husband and Leo, Amy champions Abbey's violence prevention provisions for an upcoming bill, the President drops in on a citizenship swearing-in ceremony, and Donna is appalled by a cocky new intern, Ryan (Jesse Bradford).
A brain-damaged musical savant (Dave Matthews) has seizures despite being on anti-seizure medications. When his team learns that House has entered himself for brain cancer treatment, they attempt to comfort him, but House simply turns them away.
When the flash of a secret nuclear detonation is detected over the Indian Ocean, President Bartlet calls upon his people to investigate which nation now has the atomic bomb. As conventional thinking favors Iran, the President orders bombers into the air to destroy that nation's most likely uranium-enriched targets. Meanwhile, Josh chides NASA personnel on the future of space exploration — until he is introduced to an attractive female administrator. C.J. fumes when a combative television talk-show host, Taylor Reid, denigrates her on the air, and Will discloses to Vice President Russell that Russell is considered a buffoon by the White House staff. However, it is Russell who manages to dissuade the President from bombing Iran.
An ad by a 527 group attacking Santos for his pro-choice stand has both campaigns up in arms, trying to avoid the campaign going negative or diving into the abortion issue — a touchy subject for both candidates: Vinick because his pro-choice position risks alienating pro-life conservatives, Santos because his risks alienating pro-choice liberals, including a major pro-choice interest group who are considering endorsing Vinick. Back at the White House, Will struggles with dealing with the press as the new Communications Director, settling into Toby's old office. Josh struggles with Lou Thornton's decision to bring Donna into the campaign as a spokesperson. In the final moments, as both candidates wait backstage at the Alfred E. Smith Memorial Foundation Dinner, Santos and Vinick agree to an unexpected deal: a live debate the following Sunday.
