Foot F-Scan Movie clip
In this F-Scan film we see increased pressure on the lateral column of the foot with forces traveling to the 2nd metatarsal as they come across the foot. This is most likely due to an uncompensated forefoot varus. Note the hot spots at the 2nd and 5th met heads. This patient also has a Morton's toe which is a longer digit.
Rule #1.....never assume. Examine your clients even after F-Scans, pedographs, stop frame digital movie etc. What you see is often not the problem, rather their adaptive compensation around the parts that are working and those that are not. The problem can be local, where it appears the deficit is, or it can be elsewhere. Let the clinical examination draw the correct conclusions. For example, in this case, we could have metatarsus primus elevatus or a Rothbart Foot variant or a combination of all of the above with the FF varus.
There is nice engagement of the hallux flexors but when you see this in combination with a preceding insufficient 1st metatarsal grounding (as evidenced here of lack of "heat" under the 1st Metatarsal) a late supination attempt may be suspected to reduce a late midstance pronation vector. Again, let your clinical examination draw the correct conclusions.

The LEFT foot is drastically supinating displaying a lateral weight bearing shift all the way through toe off. This case also draws clinical inquiry into whether there is weakness of the ankle and forefoot everting muscles namely the peronei and extensor digitorum longus and brevis. * This the topic of the video we are producing because these muscles have huge implications in the cross over gait (which we have senselessly beat you all with in previous blog posts) at the lower end of the limb. The Masterclass in Gait, Part Zero. The Gait Guys Way
As promised, a live hour with us, some structured, some loose and conceptual, but it is our way. This is part "zero", free to everyone. Future classes will be pay per view. We may (likely will) parse out big pieces and portions to the 20$ folks for free, included in their monthly donation. But, do not hold us to this, we are undecided, but we did not want to hold back part ZERO, the rough intro so you can get a feel for us, and the material we are going to teach. Remember, there will only be 100 live seats when we go live (likely Wednesday nights, or Friday late afternoons Central time), and Patreon folks will get first dibs to fill seats, so check in regularly, we will post a TEXT notification here and give a weeks notice. For those who cannot get into the LIVE class where we will take LIVE real time questions, just like in a classroom, we will record the classes and harbour them on a service like youtube or vimeo behind a pay wall. When the knee hinges sideways. A clinical video case.
This is not a difficult case today, not by any means. Most people will can see what is not normal here. But there are some simple principles we wanted to highlight and remind you of that this case shows nicely.
This is a fairly typical advanced degenerative arthritic right knee and the gait that accompanies it.
Here you can see that when the gentleman steps onto the right limb the knee has a small lateral hinge moment, you can see the knee joint buckle sideways. This is not normal, the knee is supposed to hinge only forward and backwards (flexion and extension) in the sagittal plane. Here it is hinging in the frontal plane. You can easily see that after many years of abnormal stresses that the tibia has deformed into a varus bowed position. This is a great example for you engineer-type out there about long term deformation of solids.
* Deformation of Solids:
Stress: is a measure of the force required to cause a particular deformation.
Strain: is a measure of the degree of deformation.
Elastic Modulus: the ratio of stress to strain:
Elastic modulus = Stress divided by Strain or
EM= Stress / Strain
The lateral forces and hinging over time forced the tibial to varus bow which is a reactionary measure. In simplest of terms, as the bone cells (osteoclasts and osteoblasts) continued to cyclically turn over ....

Part 2 of the EHB: Bringing the Extensor Hallucis Brevis of the Foot Back to Life.
Today we show you a proprietary exercise we developed here at The Gait Guys. It was developed out of necessity for those clients who are too EHL dominant (long big toe extensor muscle) and big toe short flexor dominant (FHB). These two muscles are what we call a foot functional pair. Big toes like these will be dysfunctional and will not be able to gain sufficient purchase on the ground to produce stability and power without impacting the joint (1st metatarsophalangeal joint). Imbalances like these lead to altered joint loading responses and can be a possible predictor for premature damage to the joint over time. These imbalances are also what lead to injuries to the big toe, the arch and other areas of the foot. After all, when the big is weak or dysfunctional gait will be compensated. When imbalance at this joint occurs because of EHB weakness the medial tripod anchor (the head of the 1st metatarsal) is compromised possibly transmitting stress into the foot, arch and medial stabilizers such as the tibialis posterior for example.
This exercise is to be weaned back to less and less yellow band resistance until the EHB can be engaged on its own. Then the gait retraining must begin. Simply reactivating and strengthening the skill and muscle is not enough. The pattern must be then taken to the floor and learned how to be used in the gait cycle.
It is a process restoring gait. All too often the "Devil is in the Details".
