In the mission Finding Johnny Vincent, a few players have encountered a very strange glitch where, for some reason, after entering the asylum, the game fades out and then you're playing as the Asylum Orderly without mods.
This glitch is known to occur on PC & Wii, and while PC does have mods, the earliest trace of this glitch goes back to 2008 on the Wii....
