The President asks a reluctant Vice President (Tim Matheson) to speak at an anti-gun rally in Texas after a church shooting, while Donna goes before a Congressional committee investigating Bartlet's lack of disclosure and lies under oath when her new boyfriend asks her a personal question. Toby deals with a leaked quote that is embarrassing to the President, while C.J. meets up with a gutsy reporter and Sam ponders a plan to eliminate the penny.
Donna, Josh and Toby are stranded in Indiana when the presidential motorcade leaves a campaign stop without them, leaving Sam to staff the President alone. Leo is informed that Qumar has reopened the investigation into the death of its defense minister, Abdul ibn Shareef. In a stop in a hotel bar on their way back to the White House, Donna, Josh and Toby meet a man who is struggling with the thought of paying for his daughter's education. Sam spends the day as the President's "wide-angle lens" on policy issues. Debbie Fiderer has a second job interview with the President and her impressive ties to Charlie Young are revealed. A bombing at a collegiate swimming meet sparks a national tragedy.
After returning from jogging with his best friend, Ed (Dominic Purcell) is uncharacteristically abused by his bedridden wife, Elise (Myndy Crist). House concludes that she either has Rabbit Fever from cooking rabbits or African Sleeping Sickness, which must have been sexually transmitted since neither of them has been to Africa. They both strongly deny having an affair so House starts treatment for Rabbit Fever. When this fails he successfully treats her for Sleeping Sickness. She confesses to an affair with her husband's best friend, causing her husband to leave her.
Travis tasks Beth with unleashing Wormwood poison gas in the Miami Metro squad room, but she's stopped by Dexter, saving everyone's life. Debra has dinner with Chief Matthews where she confronts him about being implicated in the dead call girl case. When Matthews is forced to retire, she realizes that LaGuerta has been manipulating her throughout the case. Dexter stages a fake Doomsday Killer tableau in order to draw out Travis, but is captured himself and becomes a part in the final tableau, the "Lake of Fire". Debra's therapist prompts her to consider whether she may actually be in love with her adopted brother.
Six months after the season 3 finale, Dexter is now living with three kids and a wife in a new home. Along with his new son Harrison, the complexities of being a suburban father of three is making it difficult for Dexter to concentrate on his work. Dexter's lack of sleep results in a murderer being released because Dexter brought the wrong case file to court, irritating Quinn. Quinn begins a relationship with reporter Christine Hill. A recently retired Agent Lundy has returned to finally capture a shadowy serial killer known as the "Trinity Killer", who has been at large for 30 years and always kills three people every year. After a call from Rita, Dexter quickly kills the released murderer but totals his car after falling asleep at the wheel.
The international crisis concerning the terrorist abduction of President Bartlet's daughter Zoey reaches a critical point as acting President Walken orders the bombardment of Qumari terrorist camps and fences with Leo over the impact of his actions. The kidnappers issue a 24-hour deadline for the removal of American troops from Qumar, but an intelligence break brings an end to the crisis. Meanwhile, Josh fumes over his perceived notion that the Republicans will exploit and push forward their own legislative agenda, and Toby visits his newborn twins while pondering what will happen.
A morbidly obese man (Pruitt Taylor Vince) is found in a coma after a fire accident and is admitted to Princeton-Plainsboro. Upon waking up, he demands to be discharged, refusing to be tested for any disease possibly caused by his weight.
When new polling puts Santos and Vinick neck-and-neck nationwide, the Santos campaign staff are elated – Josh and Donna so much so that they end up sharing a kiss, which forces them to confront their feelings for each other. Vinick, on the other hand, has a cold, and is feeling pressure from the RNC to make a rightward turn in his campaign and play to the Republican base. This makes Bruno nervous and afraid he's going to be fired. President Bartlet summons both candidates to the White House so he can advise them that he's ordering tens of thousands of U.S. troops into Kazakhstan. Before he meets with either Presidential candidate, he meets with Leo McGarry, whom he has also called to the White House. Bartlet's meeting with Leo is more personal as he shares his concerns about the consequences of his decisions, with Leo back in his old role as the President's closest confidant. This would be John Spencer's last appearance.
A student (Vicellous Reon Shannon) who just graduated from Princeton experiences severe spasms at a graduation party. Meanwhile, House's parents drop by but he is reluctant to see them, igniting curiosity among the hospital staff.
The staff participates in "Big Block of Cheese Day," Toby is assigned to speak with a group of unruly anarchists protesting the WTO, C.J. meets with The Organization of Cartographers for Social Equality, and a friend of Donna asks Sam to consider a pardon request for an alleged Cold War spy.
