House receives a visit from an ex-girlfriend, Stacy Warner, who seeks his help for her husband, Mark. In the meantime, Cuddy forces House to give a lecture to medical students on diagnosing patients and presents three scenarios, each with different reasons for their leg pain (with guest star Carmen Electra).
When a British passenger aircraft is accidentally shot down over Iran, causing an international crisis, C.J. battles with the First Lady over how much to let Bartlet's MS affect his schedule; Toby and constitutional scholar Lawrence Lessig (Christopher Lloyd) work with Belarusian diplomats on a new constitution.
Dexter refuses Debra's request to kill Hannah. Meanwhile, Isaak escapes his police surveillance and attempts to kill Dexter again, but fails. Astor and Cody come to visit Dexter. Batista buys a restaurant. Feeling he is out of control with trying to avenge Viktor's death, the Koshka Brotherhood puts a hit on Isaak. Also, LaGuerta finds a potential link between Dexter and the Bay Harbor Butcher. Elsewhere, Debra learns that Dexter and Hannah are seeing each other and, in conversation with him, reveals more than she wanted. Isaak reveals to Dexter that the reason why he is trying to kill Dexter for Viktor's murder is that Isaak and Viktor were lovers.
When Dexter is saddled with an unwanted conspirator, he is forced to make some hard decisions about her fate when Lumen wants in on his killing routine to find the men who abducted and raped her. Meanwhile, Dexter wonders if Rita's death has awakened a darkness inside baby Harrison. Due to Quinn's recent suspension, Debra works the Santa Muerte case alone, leaving Quinn time to enlist an old friend, Stan Liddy (Peter Weller) to look into Dexter.
Elections are underway in Kazakhstan, but Russia and China teetering on the brink of an oil war in Central Asia is the least of Bartlet and C.J.'s concerns as they face a nuclear reactor in California on the verge of a meltdown. While agonizing over whether to evacuate nearby citizens and release radioactive steam into the atmosphere, the Santos and Vinick campaigns stare each other down, trying to avoid being the first to turn the crisis into a political issue – all the more complex as, decades ago, Vinick lobbied for the plant's construction, while Santos has spoken about the dangers of nuclear power. When the news comes out, the election becomes, in Josh's words, "too close to call."
Leo tries to convince President Bartlet of the importance of supporting a missile defense plan, while Lord John Marbury is appointed British ambassador to the United States. Toby and Sam clash over a speech the President gives to an environmental group. C.J. tries to talk a comedian out of embarrassing the Bartlet administration.
Bartlet engages both Sam and Toby in intricate chess matches that mirror the wily game of brinksmanship that Bartlet is playing with the Chinese, who are conducting war games in the Taiwan Strait. The small New England town of Hartsfield's Landing kicks off voting in the New Hampshire primary, and Josh is lobbying for every vote he can get. A prank war between C.J. and Charlie turns somewhat destructive.
Under constant surveillance by federal agents, Dexter is unable to dispose of his latest victim's remains. He apologizes to Rita, admitting that his affair with Lila was a mistake, but she is still hesitant to forgive him, as Lila retaliates by pursuing Angel. After finding Dexter's collection of blood slides, Doakes seeks the advice of an old friend in Haiti to analyze them. The department begins to suspect Doakes' involvement in the Bay Harbor Butcher case, but cannot find him for questions. Debra and Lundy spend the night together and contemplate making their relationship public.
House takes the case of Adam (Braeden Lemasters), a 10-year-old severely autistic boy, who screams loudly for no apparent reason. Cuddy makes a minor change to House's office and he refuses to use it until it is returned to its original state; thus, he finds himself wandering the hospital in need of a temporary office. Meanwhile, Ali (Leighton Meester) still has a crush on House and is becoming a nuisance.
While C.J. gets used to being tailed by Special Agent Donovan, the President must deal with a terrorist threat on a military installation that gets closer and closer to home and leads to a frightening revelation. Meanwhile, Toby plays hardball with network TV executives who want to cut national convention coverage, and Sam's plan to keep dirty tricks out of the Presidential campaign blows up in his face.
