Dr. Shawn Allen of The Gait Guys speaks about proper stabilization of the medial foot and arch. Muscle specifically discussed are a team: FHB (flexor hallucis brevis), AbDuctor hallucis, and tibialis posterior. He discusses the functional anatomy, normal and pathologic movement patterns of the arch and first ray complex and big toe (hallux). His foot's ability to show the optimal patterns for the arch and hallux are excellent examples. So what do we see here?
This gentleman has an atrophied gastroc/soleus on the left from an injury. He compensates by increasing thigh flexion on the left to clear the leg. Because he has lost gastroc/soleus strength on the left (the lateral gastrocis an important inverter of the heel after midstance and important component of rearfoot supination), the rearfoot everts more, allowing more midfoot pronation. This collapse of the midfoot brings his weight more medially, so he shifts his pelvis laterally (to the left) to keep his center of gravity over the foot.
Fix?
Make client aware of what is going on.
make sure gastroc/soleus complex strength and function is maximized through muscle work, acupuncture, muscle activation, functional gait exercise

This is a case of internal tibial torsion. Watch the right limb. We see a decreased progression angle of the feet, Right greater than the left, a "crossing over" gait; and bilateral toe off in supination, Right greater than the left. This can impair a persons ability to engage the lumbopelvic stabilizers adequately, impair patellofemoral tracking, and generate a host of compensatory motor patterns that are sub-optimal.
