The problem arises from the inability of the mitochondria to add iron to protoporphyrin in heme synthesis, because protoporphyrin is decreased due to heme synthesis defect which will lead to decreased synthesis of hemoglobin and therefore anemia.
Zinc supplements can cause copper deficiency and appearance of ringed sideroblast as well.
