House treats a U.S. Marine (Marc Blucas) returning from Iraq, who has symptoms consistent with Gulf War Syndrome. However, after House has a dream about the Marine despite having never met him before, the case becomes more complicated, alongside a physical problem that House has to overcome.
Senior staffers clash with newly hired political operative Bruno Gianelli and his aides Connie and Doug. Josh is upset because Leo won't let him "wave off the FDA" on RU-486. C.J. contemplates her future at the White House. President Bartlet makes it clear he values both Bruno's team and his senior staff and gets them focused on running a successful re-election campaign as allies rather than adversaries.
Billionaire entrepreneur Edward Vogler (Chi McBride) donates $100 million to Princeton-Plainsboro, officially becoming the new Chairman of the Board. Vogler intends to turn the clinic into a profitable venue for his biotech venture and also plans to eliminate House's financially draining department for good. Meanwhile, a businesswoman (Sarah Clarke) has it all – perfect life, perfect body, perfect job – until she finds herself inexplicably paralyzed. When he diagnoses her condition, House must risk his job and his medical license to save her.
A young woman's (Jayma Mays) health becomes a question of ethics when she is unable to sleep for ten days. It is not until House discovers she will need a liver transplant that he also uncovers some vital information about her and her partner Max. Meanwhile, Cameron accuses Foreman of plagiarism when an article he authors appears remarkably similar to one of hers.
The staff debates whether to counter a fast-rising Republican presidential candidate's verbal assault on affirmative action at the Iowa caucuses and Josh postpones his tropical vacation with women's rights advocate Amy Gardner to defuse a risky situation in Vieques, Puerto Rico. C.J. surprises Toby with her lack of enthusiasm for affirmative action, while Sam has to meet with a UFO crackpot per Leo's orders. Toby and President Bartlet clash over the President's mental state and relationship with his father.
For most people, Thanksgiving is a time for traditions and family. But for Dexter, it's an opportunity to get closer to his most dangerous adversary yet. As Dexter gains insight into Arthur's psychology by studying those closest to him, he finds himself drawn into a bizarre and twisted world. Meanwhile, Rita has her hands full preparing dinner for a packed house, including a few unexpected guests. Batista gets closure on an old case, while Debra uses the holiday to continue her investigation of the Trinity Killer, a pursuit which threatens her brother's safety and brings to light surprising revelations about someone else close to her.
Everyone involved with the election is stunned when Leo McGarry has died of a massive heart attack. During an exhausting and adrenaline-filled night, the electoral votes come in as the Santos and Vinick campaigns wait anxiously. C.J. breaks the news of Leo's death to President Bartlet, and the news hits the airwaves less than an hour before polls close on the West Coast. After Santos and Vinick win their home states of Texas and California respectively, the two campaigns agonize over whether to mount legal challenges before final results are announced in Nevada and Oregon. In the end, both states break for Santos, who gives his election night speech as the President-elect.
House is placed under a court order to determine what is ailing a mobster (Joey Arnello, played by Joseph Lyle Taylor) due for federal testimony and the Witness Protection Program. The witness's brother, a lawyer, works against the team and the testimony when his brother is diagnosed with Hepatitis C. Cuddy continues to battle Vogler over House's importance to the hospital.
The President and his senior staff deal with major problems – a hostage standoff between the FBI and a group of survivalists in Idaho, a pre-season hurricane headed toward the Atlantic Coast, and the threat of a crippling nationwide trucking strike by the Teamsters – all while preparing for a state dinner to honor the new President of Indonesia, whose behavior is distant and uncommunicative. Toby requests a favor from a senior aide to the Indonesian president; the aide not only refuses but also lectures Toby about human rights, and for being a hypocrite. At the state dinner, the First Lady, Abigail Bartlet, makes her first appearance, and Sam sees Laurie, who is working as an escort for one of the attendees. In a predictable move to evade the storm, an aircraft carrier task group of the US Navy, including two cruisers and two destroyers, gets underway from Norfolk, Virginia, and heads into the Atlantic Ocean; however, the hurricane makes a surprise course change and moves directly toward and across the naval vessels. With difficulty, the President chats via radio with a young petty officer in the radio shack aboard a small ship in the task group.
The Miami Metro Police Department, led by FBI Special Agent Frank Lundy, begins the hunt for the "Bay Harbor Butcher". Both Dexter and the police are hunting for Little Chino, and Dexter is determined to kill him properly given a second chance. Debra and Dexter struggle to overcome memories of the encounter with Brian that led to his death. After Paul's funeral, Rita confronts Dexter about his involvement in Paul's death, and he admits to having an addiction, which Rita infers to be to drugs. LaGuerta shows surprising support for new boss, Lieutenant Esmee Pascal (Judith Scott).
