In today’s episode I breakdown a common skin infection - Genital Herpes.
Genital herpes is caused by the virus known as Herpes Simplex Virus (HSV), of which there are two types, 1 and 2. HSV is a common Sexually Transmitted Infection, affecting up to 1 in 8 Australians over the age of 25.
Typically HSV 1 tends to affect the mouth area causing cold sores, and HSV 2 favours the genitalia. However due to changes in sexual practice, the two can be reversed.
The first episode of HSV 2 can be proceeded by a prodrome of feeling unwell, having fevers, chills, glandular swelling and general fatigue. Thereafter there may be some blisters and ulceration which can be sore, itchy and painful.
Usually patients will present to the doctor for treatment. This will usually encompass an examination, including a swab and medication which can take the form of an anaesthetic cream or anti-viral tablets. These will help to reduce the symptom duration. Usually the first episode will last up to 3 weeks.
HSV is transmitted through close skin to skin contact and so it is vital if you are affected not to engage in this practice until symptoms have resolved on the advice of your doctor. Transmission can also occur in asymptomatic people and so the best way to help prevent infection is to use barrier methods.
HSV remains dormant in the body after initial infection and can reoccur especially if your immune system is low or you are stressed. Typically recurrent episodes tend to be less severe and shorter in duration.
It is therefore important to lead an active and healthy lifestyle to reduce your chances of getting a recurrent outbreak.

The novel coronavirus (Covid-19) originated in China, Hubei Province and has spread to most countries across the world.
The disease causes respiratory illness (like the flu) with symptoms such as a cough, fever, and in more severe cases, difficulty breathing.
Coronavirus disease spreads primarily through contact with an infected person when they cough or sneeze. It also spreads when a person touches a surface or object that has the virus on it, then touches their eyes, nose, or mouth.
You can protect yourself by washing your hands frequently, avoiding touching your face, and avoiding close contact (1 meter or 3 feet) with people who are unwell.
Many countries have gone into 'lockdown' strongly advising citizens to stay home unless essential, such as shopping for groceries or seeing the doctor.
It is imperative that if you yourself think you have symptoms of Coronavirus that you call ahead to a local testing centre or doctor so that they can be ready to receive you if required.

WHAT IS CERVICAL SCREENING?
In order to understand this better, let's go through a little anatomy. The cervix is found at the top of the vagina and forms the entrance of the womb - the organ in females that allows us to carry children. By having a cervical screening test it helps to detect any abnormal cells that may go on to causing cervical cancer.
THE TEST
You may have already heard of the PAP smear - The PAP smear preceded the cervical screening programme. The test consisted of collecting, staining and microscopic examination of the cells from the cervix. If abnormal cells were found they were removed to stop cervical cancer developing. The PAP test relied on cell changes being visible under a microscope and so it was recommended to have this every 2 years.
It was later discovered that HPV causes cervical cancer and the first HPV test was invented 15 years later. And it is this principle that we use today which forms the basis of the cervical screening test. Cells are taken from the cervix in the same way as the traditional PAP smear but instead of looking for cell changes it looks for the types of HPV which can cause cells to change and in some cases cause cervical cancer. HPV is the cause of more than 99% of cervical cancers.
WHAT IS HPV?
HPV, otherwise known as Human Papilloma Virus, is very common, in fact it's so common that many of us including men, will have it at some point in our lives and not know about it as it has no symptoms. There are a lot of different types of HPV infections - to be precise - there are over 100 of which at least 14 are considered high risk and can cause cancer.
HPV is spread from genital skin to skin contact and it is considered the most common sexually transmitted infection. Most HPV infections will be naturally cleared up by your own immune system within one or two years without causing problems. In rare cases your body may not clear it and so your doctor may refer you onwards for further management.

CANNULA
A cannula is a blunt plastic tube that is introduced to the skin using needle. As the cannula is blunt, it is less likely to penetrate through important vessels and is less traumatic to underlying tissues, this means there will be less swelling and bruising.
A cannula also allows for fewer injection sites which is more comfortable for the patient. The use of cannula can be quicker than a needle but this does depend on the area being treated.
CHEEK FILLER WITH CANNULA
For the cheek filler treatment, first an incision is made with a 25 gauge needle, this provides the opening for the cannula to be inserted. The needle is then removed and the cannula passed through the entry point to below the skin surface. The cannula can re-enter in the same injection site and so allowing for filler to be deposited in multiple areas with just one entry point.
NEEDLE
Needles are great because they give you more control on the areas you're injecting, allowing you to give small precise deposits of filler. That being said, you will require more injection sites.
TEAR TROUGH FILLER WITH NEEDLE
First up the area is cleansed and the safety zones to be injected are marked on the face. Several ultra fine needles are used to deposit small amounts of filler along the boney prominence of the face. After the injections the filler is massaged into place.
PROS AND CONS
When deciding which method to use it depends on a number of factors.
- Needles are considered to be more precise and can be injected deeper into the issue, but do require more injections to be made, which for the patient can be the most wince provoking part of the treatment.
- The patients experience and expectations play another factor here, most patients may have experience or at least know what to expect from a needle. With the cannula, while generally more comfortable, you can still feel it moving under the skin, which can present a new and un-natural experience. So the patients experience level and preference is important to consider.
- The areas being treated especially in regards to the surrounding anatomical structures need to be taken into careful consideration. As the cannula is blunt, it is less likely to penetrate through important vessels and is less traumatic to underlying tissues, this means there will be less swelling and bruising.

Today in the clinic, we have Katie who has naturally beautiful lips, however she's looking for "much bigger lips". For this treatment I'll be using a thicker product than shown in previous videos, to achieve that dramatic volumization.
The treatment consisted of using 1ml of HA (Hyaluronic Acid) based filler, which is a temporary filler that generally lasts 6 months. The technique performed in the treatment is linear threading, which consists of making multiple incisions and deposits along the skin depression and I find provides the most comfort for patients.
In my opinion post-treatment the results were fantastic. Her lips looked full and volumized with minimal bruising. While there was some inflammation in the after shots which will reduce by around 10% over the next few days, looking at the before and after shots you can see a dramatic change in the volume of her lips. Stunning!

In this video we listen to the heart of a patient who has a diagnosis of diastolic dysfunction and a heart murmur.
WHAT IS DIASTOLIC DYSFUNCTION?
Diastolic dysfunction occurs when the left ventricle gets stiffer and is not able to accept blood return in a normal fashion. It can lead to high blood pressure and even heart failure thus requiring regular monitoring and management.
This lovely patient also has a heart murmur that you can clearly hear with the stethoscope. We take a listen to her heart at rest and during exercise.

How much salt are you eating? In today's episode of Dr Nora, I take you through why we need salt in our diet, how much we need and what happens if we have too much.
SALT
Salt is a chemical compound made up of sodium chloride. Our bodies need sodium to maintain fluid balance and volume which is thought to be less than half a teaspoon a day. However, if we have too much sodium in our bodies it can lead to a number of negative health impacts.
Current recommendations allow the average Australian adult to consume no more than 5g of salt a day or 2300mg of sodium per day. Unfortunately it's been reported that most adults consume 9g of salt per day which is almost double the recommendation.
HEALTH IMPACTS
Consuming too much salt over prolonged periods can cause a raised blood pressure. When blood pressure is persistently elevated and not treated this can cause strain on the vital organs of the body such as the heart, increasing the risk of heart attacks and in turn increases the risk of stroke.
HIGH SALT FOODS
Foods that are generally higher in salt include, deli meats, cheese, bread, processed meals, canned fruit or vegetables, sauces and salad dressings. Take a look at the nutritional labels, there is a line that illustrates how much sodium is in a food per 100g. A low salt option would be a food that contains under 120mg of sodium per 100g and a high salt option would be a food that contains over 400mg of sodium per 100g. It's these higher salt foods that you should limit at much as possible.
HOW TO REDUCE DAILY SALT
Other than being mindful of the nutritional labels, actively seek labels that say 'reduced salt' or 'low in salt'. These options will generally have less salt than the original item but still maintain the same flavour. It's important to know that even though they generally contain less salt, they still contain salt none-the-less, so be wary of your consumption.
You could also try 'light salt' options which are generally are made up of both potassium and sodium chloride, in these options, there is more potassium (another chemical element) than sodium and hence allowing it to have a lower sodium content.
Instead of using salt in your food preparation you could use aromatic flavours, such as spices or curry leaves to help maintain that kick in your food.
