Dans A Plague Tale : Innocence, le joueur incarne Amicia de Rune, une adolescente âgée de 14 ans, accompagnée de son petit frère de 5 ans, Hugo, deux orphelins livrés à eux-mêmes alors que la guerre et la peste font rage. Bien qu'étant du même sang, les deux enfants se connaissent peu puisqu'Amicia a été élevé par son père, Robert de Rune, qui lui a appris à chasser, tandis qu'Hugo, atteint d'une mystérieuse migraine, est gardé par sa mère Béatrice de Rune dans sa chambre. Ils vont devoir survivre et ne compter que sur eux-mêmes alors qu'ils sont poursuivis par l'Inquisition dirigée par le seigneur Nicholas qui en a après Hugo qui le tient pour responsable de l'apparition de la Peste Noire par le biais de sa maladie. Et comme si cela ne suffisait pas, ils sont également cernés par une multitude de rats prêts à les dévorer.
